Wrist Sprains

A wrist sprain is an injury to one or more of the ligaments that connect the carpal bones of the wrist. Wrist sprains are extremely common in sports and can range from a mild stretch (Grade I) to a complete ligament tear (Grade III). Because the wrist contains numerous small ligaments, accurate diagnosis is essential to prevent long-term instability.

Grading and Common Ligaments Injured

Grade I sprains involve microscopic ligament tears with no instability. Grade II sprains are partial tears with some laxity. Grade III sprains are complete tears with significant instability.

The scapholunate ligament is the most important wrist ligament — a complete tear leads to progressive carpal instability and arthritis if untreated. TFCC (triangular fibrocartilage complex) injuries are also common and cause ulnar-sided wrist pain.

Treatment

Grade I and II sprains are treated with immobilization in a wrist splint or cast for 4–6 weeks, followed by rehabilitation. Most mild sprains heal well with conservative care.

Grade III tears causing instability require surgical treatment — arthroscopic or open ligament repair for acute injuries, and ligament reconstruction or salvage procedures for chronic instability.

Common Symptoms

  • Wrist pain after a fall or twisting injury
  • Swelling and bruising
  • Tenderness over specific carpal bones
  • Decreased grip strength
  • Pain with weight-bearing through the wrist
  • Clicking or clunking with wrist motion (instability)

Treatment Options

  • Wrist splinting or casting for Grade I–II sprains
  • NSAIDs and activity modification
  • Arthroscopic ligament repair for Grade III tears
  • Scapholunate ligament reconstruction
  • TFCC repair or reconstruction
  • Structured wrist rehabilitation
