Elbow Injuries

The elbow is subjected to tremendous forces during throwing, swinging, and contact sports. Sports-related elbow injuries encompass a spectrum of conditions — from ligament sprains and nerve compression to osteochondral defects and valgus extension overload syndrome — that require specialized evaluation and treatment.

Common Sports Elbow Injuries

Valgus extension overload (VEO) syndrome occurs in throwing athletes when the posteromedial olecranon impinges against the olecranon fossa during the follow-through phase, causing posterior elbow pain and osteophyte formation.

Osteochondritis dissecans (OCD) of the capitellum is a condition in which a segment of cartilage and underlying bone loses its blood supply, most commonly in young gymnasts and throwing athletes. Loose bodies from OCD can cause locking and catching.

Treatment

Many sports elbow injuries respond to rest, activity modification, physical therapy, and a structured return-to-throwing program. Medial epicondyle apophysitis in young athletes is managed conservatively with rest and technique correction.

Surgical treatment is indicated for unstable OCD lesions, symptomatic loose bodies, VEO with osteophytes, and UCL tears requiring reconstruction.

Common Symptoms

  • Medial or posterior elbow pain with throwing
  • Locking or catching in the elbow
  • Loss of full elbow extension
  • Pain at the end of the throwing motion
  • Tenderness over the medial epicondyle
  • Decreased throwing velocity or control

Treatment Options

  • Rest and activity modification
  • Physical therapy and return-to-throwing program
  • Arthroscopic loose body removal
  • Arthroscopic OCD fixation or debridement
  • Arthroscopic posterior osteophyte debridement
  • UCL reconstruction (Tommy John surgery)
  • Technique and training load modification
